Screening and Assessment Tools

The assessment and screening tools available are often used by your provider(s) during the initial intake process and occasionally as an additional screening tool throughout your treatment. During your appointment, your provider may request that one or more of the forms below be completed. At this time, you will be provided a hard copy of the needed documents prior to leaving your appointment.  Your provider will also be able to answer any questions you may have regarding the forms.

Please DO NOT COMPLETE these forms prior to your intake appointment, your provider will need to meet with you prior to determining which screening tools are appropriate, if any.
